What are the appropriate codes to be used for reimbursement?

Effective January 1, 2004 the following are codes for reimbursement:

67912    Correction of lagophthalmos

HCPCS Medicare Code

L8610   Other prosthetic procedure devices - ocular

L8499    Unlisted procedure for misc. prosthetic services
